On a daily basis C&C HOMECARE provides an extensive array of home medical equipment, respiratory and rehabilitation products to patients being discharged from Hospitals, Rehab Centers, and Skilled/Sub-Acute Care Centers. Also, we provide products and services, through our specialized programs, via direct

contract to over 40 skilled nursing facilities in the New York metropolitan area. This sets us apart from most homecare companies and provides the patient with a continuity of care from any facility to their home regardless of their equipment requirements.

C&C HOMECARE is extremely well versed in the needs of a hospice program such as yours. We currently have contracts and provide complete durable medical equipment, disposable supplies and services to several different hospice programs.
